目的:探讨应用LiF(Mg, Cu, P)热释光剂量计(TLD)和Alderson Rando仿真人模体模拟测量乳腺癌改良根治术后容积旋转调强放疗(VMAT)中不同定位方式对体表吸收剂量的影响,为临床治疗提供参考。方法:应用直线加速器6 MV X射线,对TLD剂量计进行剂量标定、角响应实验。基于仿真人模体模拟乳腺癌改良根治术后VMAT放疗,测量3组不同情况下的体表吸收剂量。对照组:模拟乳腺托架定位,不覆盖组织补偿物(Bolus);实验组1:模拟乳腺托架定位,覆盖0.5 cm厚度的Bolus;实验组2:体板联合热塑体膜进行定位。结果:对照组:模体表面无任何覆盖物时测得体表吸收剂量为94.4~99.9 cGy,占处方剂量的47.2%~50.0%;实验组1:模体表面覆盖0.5 cm厚度的Bolus时测得体表吸收剂量为179.4~184.6 cGy,占处方剂量的89.7%~92.3%;实验组2:覆盖热塑膜时测得体表吸收剂量为150.4~156.8 cGy,占处方量的75.2%~78.4%。结论:3组不同情况下的体表吸收剂量差异较大。在临床治疗中,热塑膜能够提高皮肤表面剂量,但存在较大的不确定性。应合理利用热塑膜联合体板减少呼吸幅度的同时合理运用Bolus来提高体表吸收剂量,从而达到治疗效果最优化。
Abstract:
Abstract: Objective To investigate the effects of positioning methods on the superficial absorbed dose in volumetric modulated arc therapy (VMAT) following radical mastectomy using LiF (Mg, Cu, P) thermoluminescent dosimeter (TLD) based on anthropomorphic phantom, thereby providing some reference for clinical treatment. Methods Linear accelerator 6 MV X-ray was used for the dose calibration and angular response measurement of TLD. The superficial absorbed dose of the Alderson Rando phantom in VMAT following radical mastectomy was measured under 3 positioning conditions, namely breast bracket in control group, breast bracket covered with Bolus of 5 mm in experimental group 1, and body plate combined with thermoplastic film in experimental group 2. Results The measured superficial absorbed dose in control group and 2 experimental groups were 94.4-99.9, 179.4-184.6 and 150.4-156.8 cGy, accounting for 47.2%-50.0%, 89.7%~92.3% and 75.2%~78.4% of the prescription dose, respectively. Conclusion The superficial absorbed dose differs significantly under 3 positioning conditions. In clinical practice, thermoplastic film is able to improve the superficial dose, but there exists greater uncertainty. Bolus should be rationally used to reduce the respiratory range and improve the superficial absorbed dose, so as to achieve the optimal treatment outcome.
