非痴呆型血管性认知障碍(VCIND)是血管性认知障碍的早期可干预阶段,其早期精准识别对延缓疾病进展意义重大。静息态功能磁共振成像(fMRI)凭借无创优势,成为解析VCIND脑功能网络异常的关键手段。本文聚焦静息态fMRI多模态分析方法,系统综述了其在VCIND研究的核心进展:局部脑功能分析揭示默认模式网络(DMN)等核心脑区自发活动强度与局部协同性异常;功能连接(FC)分析明确DMN、额顶网络(FPN)跨区域连接损伤及干预后的可塑性变化;复杂网络分析阐释患者脑网络拓扑结构整合效率下降特征。该多维度分析体系为VCIND神经生物学机制阐释提供多层次客观依据,挖掘出潜在临床生物标志物,为疾病早期精准识别与干预治疗提供关键技术支撑。本文进一步探讨了技术创新与多模态数据融合在VCIND精准诊疗中的应用前景,为延缓疾病进展、制定个体化干预策略提供新方向。
Abstract:
Abstract: Vascular cognitive impairment with no dementia (VCIND) is an early modifiable stage of vascular cognitive impairment, and its precise early identification is of great significance for delaying disease progression. Resting-state functional magnetic resonance imaging, by virtue of its non-invasive advantage, has become a pivotal tool for deciphering abnormalities in the brain functional networks of VCIND. This review focuses on the multimodal analytical approaches of resting-state functional magnetic resonance imaging, and systematically summarizes their core advances in VCIND research. Specifically, local brain function analysis reveals abnormalities in spontaneous activity intensity and local coordination in core brain regions such as the default mode network. Functional connectivity analysis clarifies the cross-regional connectivity damages in the default mode network and fronto-parietal network, as well as plastic changes after intervention. Complex network analysis elaborates on the characteristic decrease in the integration efficiency of the brain network topological structure in patients. This multidimensional analytical system provides multi-level objective evidence for the elucidation of the neurobiological mechanisms of VCIND, identifies potential clinical biomarkers, and offers key technical support for the precise early identification and intervention of the disease. Furthermore, the application prospects of technological innovations and multimodal data fusion in the precision diagnosis and treatment of VCIND are discussed, thereby providing a new direction for delaying disease progression and formulating individualized intervention strategies.
