 目的:探讨不同扫描条件对CT值到相对电子密度(HU-RED)转换关系及放疗计划剂量计算的影响。 方法:借助CT模拟定位机和电子密度模体,分析不同扫描条件下各组织替代物的CT值,建立对应的HU-RED转换关系。随机选取头部、胸部、腹部、盆腔肿瘤患者各8、8、6、6例,分别设计放疗计划。保持放疗计划各参数不变,选择不同电压下的HU-RED转换曲线,进行剂量计算。借助临床靶区(D2、D50、D95、D98)和危及器官(Dmean、Dmax),分析比较扫描电压的改变对剂量分布的影响。 结果:扫描层厚和扫描电流对HU-RED转换关系影响可忽略,但扫描电压对其影响较大;头颈部和盆腔肿瘤靶区(D2、D50、D95、D98)剂量有显著性差异(P≤0.029);胸部和腹部肿瘤靶区(D50、D95、D98)剂量有显著性差异(P≤0.043);脑干、患侧肾、小肠、双侧股骨头、脊髓、膀胱的平均剂量及脊髓的最大剂量有显著性差异(P≤0.036)。 结论:CT模拟定位机扫描电压的改变可显著影响剂量的计算,因此治疗计划中需要选用与CT扫描电压相一致的HU-RED转换曲线进行剂量计算。
Abstract:
 Abstract: Objective To study the effects of different scanning conditions on the conversion of CT values to relative electron density (HU-RED) and dose calculation of radiotherapy plan. Methods With the use of CT simulator and electron density module, the CT values of various tissue substitutes under different scanning conditions were analyzed and the corresponding HU-RED conversion relationship was established. Radiotherapy plans were designed for 8, 8, 6 and 6 cases of head, thoracic, abdominal and pelvic tumors. The parameters of radiotherapy plan remained unchanged, and HU-RED conversion curves under different voltages were selected to perform dose calculation. The D2, D50, D95, D98 of target areas and the Dmean, Dmax of organs-at-risk were analyzed to evaluate the effects of scanning voltage on the dose distribution. Results Slice thickness and scanning current had trivial effects on the HU-RED conversion. However, scanning voltage has a great influence on the HU-RED conversion. There were significant differences in the D2, D50, D95, D98 of head and neck and pelvic tumors (P≤0.029), and the D50, D95, D98 of thoracic and abdominal tumors, and the average dose of brain stem, ipsilateral kidney, small intestine, bilateral femoral head, spinal cord, bladder, and the maximum dose of spinal cord (P≤0.036). Conclusion The scanning voltage of the CT simulator can significantly affect the dose calculation. Therefore, the HU-RED conversion curve which is consistent with the CT scan voltage should be used for dose calculation.
