目的:为提升基于桡动脉脉搏波的无创血管功能评估效能,构建融合多时域血流动力学参数的综合指标-血流动力学指数(HVI),用于表征与年龄相关的血管功能变化。方法:在恒定接触压力下采集青年、中年与老年3组受试者的桡动脉脉搏波信号,通过三角形血流波形分解并计算反射波幅值比(RM)、反射指数(RI)与重搏波比(Rd),并基于主成分分析(PCA)构建综合反射指数HVI。采用ROC曲线评估各指标的判别效能,并通过5折交叉验证检验HVI的泛化能力。结果:RM与RI随年龄增加而升高,Rd在老年组显著增大,反映血管顺应性下降和反射增强。HVI在3组间差异显著,青年vs.老年组的AUC为0.978,高于单一指标;在青年vs.中年及中年vs.老年比较中亦保持较高区分性能。交叉验证结果(平均AUC=0.914)表明HVI具有良好稳定性与泛化能力。结论:HVI能有效整合多项反射信息,比传统单参数更全面反映年龄相关的血管功能变化,具有作为血管老化量化指标的潜力。其应用定位在于描述血管功能状态,而非年龄分类工具,可为未来的血管老化监测与风险分层研究提供方法学基础。

Abstract: Objective To develop a composite hemodynamic index termed hemodynamic vascular index (HVI) that integrates multiple time-domain parameters to characterize age-related vascular functional changes, thereby enhancing the performance of noninvasive vascular function assessment based on radial artery pulse waves. Methods Radial artery pulse wave signals from young, middle-aged, and elderly subjects were collected under constant contact pressure. Reflection magnitude (RM), reflection index (RI), and re-dicrotic ratio (Rd) were obtained through triangular flow waveform decomposition, and HVI was developed using principal component analysis. Receiver operating characteristic (ROC) curve analysis was performed to evaluate the discriminative performance of each parameter, and 5-fold cross-validation was used to examine the generalization ability of HVI. Results RM and RI increased progressively with age, while Rd was markedly elevated in the elderly group, reflecting reduced arterial compliance and enhanced wave reflection. HVI differed significantly among the 3 groups. HVI achieved an AUC of 0.978 in distinguishing young from elderly subjects, which was higher than any single parameter, and also maintained strong discriminative performance in the young vs middle-aged and middle-aged vs elderly comparisons. Cross-validation yielded a mean AUC of 0.914, demonstrating favorable stability and generalizability of HVI. Conclusion HVI can effectively integrate multiple reflection-related features and provides a more comprehensive representation of age-related vascular functional alterations than traditional single parameters, with the potential as a quantitative indicator for vascular aging. Positioned as an indicator characterizing vascular functional status rather than an age-classification tool, HVI offers methodological support for future studies on vascular aging monitoring and cardiovascular risk stratification.
