摘要:
目的:探讨动态增强磁共振成像(DCE-MRI)对HER-2低表达乳腺癌患者腋窝淋巴结转移(ALNM)的诊断价值。方法:选择在徐州医科大学附属医院接受治疗的297例HER-2低表达乳腺癌患者为研究对象,根据是否存在ALNM分为ALNM组(n=71)和非ALNM组(n=226)。所有患者均接受DCE-MRI检查,收集并分析动态增强参数。通过Logistic回归分析确定ALNM的影响因素,运用ROC曲线评估DCE-MRI参数在诊断ALNM中的效能。结果:两组患者在病灶分布、TNM分期、脉管侵犯方面比较差异有统计学意义(P<0.05)。两组患者在短长径之比、血管外细胞外间隙容积分数(Ve)、容积转移常数(Ktrans)、速率常数(Kep)、表观扩散系数(ADC)、信号增强比率(SER)方面比较差异有统计学意义(P<0.05)。多因素回归分析显示,短长径之比、Ve、Ktrans、Kep、ADC、SER是影响乳腺癌患者ALNM的显著因素。通过对比得出,DCE-MRI参数联合预测的曲线下面积最大,为0.976,灵敏度为91.5%,特异度为92.9%。结论:DCE-MRI是判断HER-2低表达乳腺癌患者腋窝淋巴结是否转移的有效工具,可以为临床提供重要的诊断依据。
Abstract:
Objective To evaluate the role of dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) in thediagnosis of axillary lymph node metastasis (ALNM) in breast cancer patients with low HER-2 expression. Methods A totalof 297 breast cancer patients with low HER-2 expression treated at the Affiliated Hospital of Xuzhou Medical Universitywere enrolled and divided into ALNM group (n=71) and non-ALNM group (n=226) according to whether there was ALNM.All patients underwent DCE-MRI, and DCE-MRI derived parameters were collected and analyzed. Logistic regressionanalysis was used to identify the risk factors for ALNM, and the efficacy of DCE-MRI in diagnosing ALNM was assessedusing receiver operating characteristic curve. Results Significant differences were observed between two groups in lesiondistribution, TNM staging, vascular invasion, and most DCE-MRI derived parameters including short-to-long axis ratio, Ve,Ktrans, Kep, ADC and SER (all P<0.05). Multivariate regression analysis revealed that the short-to-long axis ratio, Ve, Ktrans, Kep,ADC and SER were significant risk factors for ALNM in breast cancer patients. Comparative analysis demonstrated that thecombination of DCE-MRI derived parameters yielded a maximum area under the curve of 0.976, with a sensitivity of 91.5%and a specificity of 92.9%. Conclusion DCE-MRI is an effective tool for determining the presence of ALNM in breast cancerpatients with low HER-2 expression, providing significant diagnostic evidence for clinical practice.
