目的:探索一种基于Halcyon平台的快速简洁的乳腺癌调强放射治疗(IMRT)计划设计方法,旨在降低乳腺癌IMRT计划设计难度。方法:回顾性选择10例包含锁骨上靶区的乳腺癌病例,在Halcyon上进行固定模式12野IMRT计划设计,勾画特定的辅助结构,使用基本固定的优化条件得到乳腺癌IMRT计划(HAL_IMRT)。并将该计划同TrueBeam临床使用的容积调强弧形治疗(TrueBeam_VMAT)计划进行比较。结果:HAL_IMRT计划患侧肺[V5]、[V10]、[V20]、[Dmean],健侧肺[V5],健侧乳腺[V5]及[Dmean]显著低于TrueBeam_VMAT计划(P<0.05);靶区方面CI、HI、[D2]、[D98]均不存在统计学差异(P>0.05),但HAL_IMRT计划靶区最大点剂量[Dmax]高于TrueBeam_VMAT计划(P<0.05)。结论:使用Halcyon固定模式的IMRT计划方法在满足临床剂量要求下,能够降低危及器官剂量,并且能够明显降低乳腺癌IMRT计划设计的难度。
Abstract:
Abstract: Objective To reduce the difficulty of intensity-modulated radiotherapy (IMRT) planning for breast cancer by exploring a simple and rapid method for breast cancer IMRT planning based on Halcyon platform. Methods Ten cases of breast cancer with supraclavicular target were retrospectively selected. Fixed-mode 12-field IMRT planning was carried out on Halcyon, and the specific auxiliary structure was delineated. Halcyon-based IMRT plan (HAL_IMRT) of breast cancer was obtained after the optimization using basically fixed optimization objectives. The obtained plan was compared with the conventional TrueBeam VMAT plan in clinic (TrueBeam_VMAT). Results The [V5], [V10], [V20] and [Dmean] of ipsilateral lung, the [V5 of ]contralateral lung and [the V5] and [Dmean] of contralateral breast in HAL_IMRT plan were significantly lower than those in TrueBeam_VMAT plan (P<0.05). There were no significant differences in CI, HI, [D2] and [D98 ]of target area (P>0.05), but the maximum point dose ([Dmax]) of target area in HAL_IMRT plan was higher than that of TrueBeam_VMAT plan (P<0.05). Conclusion When meeting clinical dose requirements, fixed-mode HAL_IMRT planning can reduce the dose to organs-at-risk and significantly reduce the difficulty of IMRT planning for breast cancer.
