Abstract:
To analyze the diagnostic value of 3.0T diffusion tensor magnetic resonance imaging (MR DTI) sequence in cervical spondylotic myelopathy (CSM). Methods Thirty patients with CSM admitted to the Affiliated Rehabilitation Hospital of Fujian University of Traditional Chinese Medicine were enrolled in observation group, while 30 age-matched healthy volunteers among physical examination population were selected as control group. The routine MRI sequence and DTI technology were used for scanning, and DTI technique for measuring fractional anisotropy (FA) values and apparent diffusion coefficient (ADC) values. Image post-processing and data analysis were also performed. The patients in observation group were divided into group A (n=10, simple dural sac compression), B (n=14, cervical spinal cord compression, normal signals) and C (n=6, cervical spinal cord compression, T2WI high signals) according to the results of MR plain scan. The differences of FA values and ADC values were analyzed in each group, and the diagnostic value of DTI in CSM was evaluated. Results No statistical difference was found in the FA values and ADC values of spinal cord C2-3, C3-4, C4-5, C5-6 and C6-7 segments in control group (P>0.05). There were significant differences in FA values and ADC values among control group and groups A, B and C (P<0.05). Compared with control group, groups B and C had significantly higher ADC values (P<0.05), but obviously lower FA values (P<0.05). The FA values and ADC values in group A were similar to those in control group, without statistical significance (P>0.05). From group A to C, ADC values showed an increasing trend; while FA values showed a decreasing trend, with significant differences (P<0.05). The sensitivity and specificity of FA values obtained by MR T2WI and DTI sequence and ADC value obtained by MR DTI sequence in the diagnosis of CSM were 20.00% (6/30) and 80.00% (24/30), 80.00% (24/30) and 20.00% (6/30), and 66.67% (20/30) and 30.00% (9/30). Conclusion DTI which is an early and accurate diagnosis of CSM cervical com pared with routine MRI is an effective means to show CSM lesions and observe the pathological repair process.
