- Abstract: Objective To investigate the magnetic resonance imaging (MRI) and clinical manifestations of posterior reversible encephalopathy syndrome (PRES) in patients with preeclampsia and eclampsia for improving the understanding of the disease and enhancing the levels of diagnosis and treatment. Methods The imaging and clinical data of 22 patients with preeclampsia and eclampsia combined with PRES were analyzed retrospectively. Results Among the 22 patients, 3 were preeclampsia and 19 were eclampsia. The main clinical manifestations included raised blood pressure, headache, seizures, visual impairment, mental abnormality, etc. The typical MRI image was characterized by vascular edema which was mainly located in bilateral parieto-occipital lobe. Moreover, the imaging manifestations also included isointense or hypointense signals on T1-weighted image, hyperintense signals on T2/FLAIR image, isointense or hypointense signals on diffusion weighted image, and hyperintense signals on apparent diffusion coefficient image. The distribution patterns of PRES lesions included parieto-occipital pattern in 2 cases, holohemispheric watered pattern in 9 cases, superior frontal gyrus pattern in 4 cases, partial or asymmetric pattern in 6 cases, and central-variant pattern in 1 case. After treatment, only 1 patient died. The clinical symptoms of the other 21 patients disappeared, and the prognoses were favorable. Conclusion PRES in patients with preeclampsia and eclampsia has typical clinical and MRI manifestations. The early diagnosis is helpful to improve the prognosis and therapeutic effect.
