- Abstract: Objective To explore the effects of custom invisible appliance without bracket and straight-wire appliance with metal bracket on the periodontal health of patients receiving orthodontic treatment. Methods A total of 86 patients receiving orthodontic treatment were divided into non-bracket group and metal bracket group by random number table method, with 43 cases in each group. The therapeutic effective rate, the time to straighten the tilted teeth, the time to right the twisted teeth, and the total treatment time, periodontal indexes [including gingival index (GI), gingival sulcus bleeding index (GBI), gingival sulcus detection depth (SPD), plaque index (PLI)] and patients satisfaction with treatment were compared between two groups. Results After 6 months of treatment, the total effective rate of non-bracket group was 97.67%, higher than the 86.04% of metal bracket group (P<0.05). The total treatment time, the time to right the twisted teeth and straighten the tilted teeth in non-bracket group were longer than those in metal bracket group (P<0.05). At the 6th month after treatment, there was no statistical difference in GI and SPD between two groups, without statistical significance (P>0.05) while GBI and PLI were reduced (P<0.05), and those in non-bracket group were significantly lower than those in metal bracket group (P<0.05). At the 6th month after the treatment, compared with those in metal bracket group, the patients in non-bracket group had higher subjective feelings of aesthetics, comfort, chewing function, convenience, language function and dental fixation (P<0.05). Conclusion Custom non-bracket appliance can not only enhance the effectiveness of orthodontics, but also improve periodontal indicators and oral hygiene, reduce the incidence of periodontal diseases, and finally raise patients satisfaction with orthodontic treatment, but the time for orthodontic treatment is relatively longer.
