- Abstract:
- Abstract: Objective To evaluate the safety and early and medium-term efficacy of endovascular treatment for abdominal aortic aneurysms with complex aneurysm neck. Methods The clinical data of patients with abdominal aortic aneurysms who were treated in Department of Vascular Surgery of Liuzhou Workers’ Hospital from January 2012 to October 2018 were retrospectively analyzed. Patients who were diagnosed with complex aneurysm neck and received endovascular treatment, with complete clinical data, were enrolled in the study. Results A total of 33 patients with abdominal aortic aneurysms and complex aneurysm neck were treated with endovascular treatment. The traditional endovascular repair was combined with multiple interventional techniques, including intracavitary injection of fibrin glue in 9 cases, Cuff stent implantation at proximal aneurysm neck in 12 cases, renal artery stent implantation in 8 cases, and downstream guidewire distraction technique in 4 cases. All cases were technically successful. During the follow-up period of 3 months to 7 years, 3 cases had type 1a endoleak after operation. The aneurysm cavity in 2 cases had no obvious increase during the follow-up, and the endoleak in 1 case was disappeared after another intervention therapy. In the other 30 cases, no complications such as abdominal aortic aneurysm rupture, stent endoleak, displacement and infection were observed during follow-up. Conclusion For abdominal aortic aneurysms with complex aneurysm neck, multiple interventional techniques combined with endovascular repair based on the anatomical features of aneurysm neck is proved to be safe and feasible, and the early and medium-term follow-ups indicate that the combined treatment can achieve a good therapeutic effect.
